What is EBUS and why has it replaced surgery?

EBUS combines a bronchoscope with an ultrasound probe at its tip. Guided by real-time ultrasound images, a fine needle passes through the airway wall into lymph nodes or tumours that previously required open surgery (mediastinoscopy) to reach. Accuracy is comparable, but recovery is measured in hours, not weeks.

Who needs an EBUS?

EBUS is recommended for lung cancer staging, enlarged mediastinal or hilar lymph nodes on CT, suspected tuberculosis lymphadenopathy, sarcoidosis, lymphoma evaluation, and unexplained masses sitting next to the windpipe. In many patients, EBUS provides the diagnosis that weeks of scans and blood tests could not.

Preparation, procedure and recovery

You remain NPO — nothing to eat or drink — for 4 hours before the procedure. EBUS takes 30–45 minutes under sedation; most patients describe only a sore throat afterwards and resume normal routine the next day. Biopsy samples go for pathology and, where needed, molecular testing for personalised cancer treatment.

Is EBUS painful?

No. EBUS is performed under sedation with local anaesthesia of the throat. Patients do not feel the biopsy being taken. A mild sore throat for a day is the most common after-effect.

How accurate is EBUS compared to surgery?
